The way I think about it is this: assume all code is read in
case-insensitive mode, and use #cs to mark case-sensitive code.
The #ci prefix is mostly there for symmetry.

I'm not entirely sure what the question is, but #cs and #ci work for
any readable input:
> #(a B)
#2(a b)
> #cs#(a B)
#2(a |B|)
